How to switch GLP-1 programs without a gap in treatment

Get the new prescription in hand before you cancel the old program. When you leave, the old program’s clinicians generally stop prescribing for you, and a gap in weekly doses can mean starting the dose climb again.

The program and the pharmacy are different things

People often mix up two separate pieces: the program that prescribes and the pharmacy that fills.

• The program employs the clinician who writes your prescription, and that relationship ends when your membership does. Ro’s terms say that once your last paid billing cycle is over you can no longer get its membership services, prescriptions from your provider included [1]. Found’s terms say your access to its providers and to prescription medication is suspended from the day you cancel [2].
• The pharmacy fills the prescription it holds, and a prescription can be moved from one pharmacy to another. NovoCare Pharmacy says it can transfer your prescription to a local pharmacy for pickup [3], and that a local pharmacy can transfer one to NovoCare [4]. For Zepbound, Lilly asks your prescriber to send the prescription straight to LillyDirect Pharmacy [5].

So when you join a new online program, expect its clinician to review your history and write a fresh prescription. When you only change pharmacies, a transfer may be all it takes.

Why the gap matters more than the price

The labels have short rules for missed doses. For the Wegovy injection, if one dose is missed and the next is more than 2 days away, the missed dose is taken as soon as possible [7]; if the next is less than 2 days away, it is skipped [8]. If 2 or more weekly doses in a row are missed, the label tells prescribers to restart the dose increase at a lower dose to limit stomach side effects [9]. For Zepbound, a missed dose can be taken within 4 days (96 hours); after that it is skipped until the usual day [10].

Lilly’s Zepbound page also says that anyone switching from another medicine should talk to their healthcare provider about the right approach [6]. Your new prescriber decides your dose, not the program’s sales page.

If timing is tight, overlap the memberships, not the medicines

Paying for two programs for a short stretch costs money but keeps a prescriber in charge the whole time. Do not take two of these drugs together. The Zepbound label says using it with any other GLP-1 receptor agonist is not recommended [11], and the Wegovy label says the same about other semaglutide products and other drugs in the class [12]. Ask the new clinician which day to take your first dose under the new prescription.

A simple order of steps

1. Write down your medicine, your current dose, the date of your last dose, and how many doses you have left at home.
2. Sign up with the new program and complete its medical intake. Give the clinician those details, plus a photo of your pen label or a pharmacy receipt if you have one.
3. Wait for a message that the prescription was written and sent to a pharmacy. If your insurance needs prior authorization first, LillyDirect notes that the step can take anywhere from a few days to a few weeks [19].
4. Cancel the old program before its renewal cutoff (table below) and keep the confirmation email or a screenshot.
5. If you pay cash through LillyDirect, check the refill window described below.
