Zepbound patient assistance: Lilly Cares and cheaper routes

Lilly says people with limited income and no insurance may get Lilly medicines free through the Lilly Cares Foundation, but no Lilly page that could be read says whether Zepbound is one of them. Until that is confirmed, plan around the prices you can check: LillyDirect’s self-pay price, the Zepbound savings card if your job-based plan covers it, and the Medicare GLP-1 Bridge.

What Lilly says about Lilly Cares

Lilly’s contact page describes Lilly Cares as a separate nonprofit that gives donated Lilly medicine at no cost to qualifying patients who are unemployed, or who have limited income and no insurance [1]. For details it sends you to lillycares.com [2]. That is the whole of what Lilly’s readable pages say. The Zepbound, LillyDirect and FAQ pages checked for this article do not mention Lilly Cares.

Zepbound is not on the Lilly Cares list. We read the foundation’s Available Medications page in a browser, because the site refuses automated reading: its list of all products has 26 medicines, and the only GLP-1 among them is Trulicity [17], with no Zepbound, Mounjaro or Foundayo. The page also says new Trulicity applications are being accepted again [18].

Cheapest verified ways to pay for Zepbound

LillyDirect, Lilly’s own pharmacy service, sells Zepbound single-dose vials and the multi-dose KwikPen at the same self-pay prices. A month means 28 days: four vials or one KwikPen [7].

The refill window matters. Miss it at 10 mg or more and the next fill costs $250 more [5] [6]. Lilly can also change or end the offer at any time [16].

If you have insurance

• Job-based or Marketplace plan that covers Zepbound. The savings card can bring a single-dose pen fill of one, two or three months to $25 [8], with savings capped at $1,300 a year [9]. The card ends on 12/31/2026 [12].
• Commercial plan that does not cover it. The same card offers the single-dose pen for as low as $499 a month [10], which is more than the LillyDirect vial or KwikPen at every dose in the table above.
• Medicare, Medicaid or other government coverage. The savings cards are closed to you [11]. The KwikPen self-pay card is open to cash buyers who agree not to seek repayment from any insurer or government program [13].

On Medicare: the Bridge covers the KwikPen

The Medicare GLP-1 Bridge covers Zepbound in the KwikPen only, not single-dose vials or pens [14]. If you qualify you pay $50 for a one-month supply [15]. Ask your prescriber to write the KwikPen by name, and check the rules on the Medicare GLP-1 Bridge page.
