GLP-1 nausea: how common it is, how long it lasts, what helps
Updated Sep 30, 2026. Numbers in brackets link to the source each fact comes from.
Nausea is the side effect people report most often on GLP-1 medicines, and the FDA labels show it is most common while the dose is still being raised. MedlinePlus, the National Library of Medicine’s health site, lists glutides, the drug class that includes semaglutide, among the medicines that can cause nausea and vomiting [1]. For most people in the trials it eased with time, but a few signs mean you should call your prescriber rather than wait.
How often it happened in the trials
Each label reports how many trial participants had nausea on the drug and how many had it on placebo, a dummy shot or pill. The placebo column matters: in every trial some people felt sick without the medicine. Nausea is the first row of the side-effect table on each of these labels.
| Medicine and trial group | Placebo | On the medicine |
|---|---|---|
| Zepbound 5, 10 and 15 mg, weight trials | 8% | 25%, 29%, 28% [2] |
| Wegovy injection 2.4 mg, weight trials | 16% | 44% [3] |
| Wegovy injection 7.2 mg trial (2.4 mg arm in brackets) | 13% | 39% (35%) [4] |
| Foundayo 5.5, 9 and 17.2 mg, weight trials | 10% | 26%, 34%, 35% [6] |
| Ozempic 0.5 and 1 mg, diabetes trials | 6.1% | 15.8%, 20.3% [7] |
| Mounjaro 5, 10 and 15 mg, diabetes trials | 4% | 12%, 15%, 18% [8] |
The Wegovy label has no separate table for the tablets. It says the side effects in the 25 mg tablet trial were similar in type and frequency to the injection table above [5].
These rows come from different trials with different people, so they do not rank one drug against another. The Ozempic and Mounjaro figures come from people with type 2 diabetes, not from weight-loss trials. For every rate on each label, see the pages for Zepbound, Wegovy, Foundayo, Ozempic and Mounjaro.
How long it lasts
The labels describe timing in general terms, not a number of days. On Zepbound, most nausea, vomiting and diarrhea happened while the dose was being raised and became less common later [9]. The Mounjaro label says the same [11]. On Foundayo, these symptoms were more common during the dose-raising period and then fell [10]. The Ozempic label says most reports came during dose escalation [12].
Stopping because of side effects was uncommon but real. In the Zepbound weight trials, 4.8%, 6.3% and 6.7% of people on the three doses stopped for good because of side effects, against 3.4% on placebo, and most of those who stopped did so in the first few months because of stomach and gut problems [13].
What the labels tell prescribers
- Raise the dose in steps: the Wegovy label ties its step-by-step schedule to lowering the risk of gut side effects [14].
- If a step is hard to tolerate, the Wegovy label says to consider waiting 4 weeks before the next increase [15].
- The Zepbound label says a lower maintenance dose can be considered if the chosen one is not tolerated [16].
These are the prescriber’s decisions. If nausea is making it hard to eat or drink, tell the person who prescribed the medicine before your next dose rather than skipping or changing doses on your own.
Self-care tips from MedlinePlus
This is MedlinePlus’s general advice for nausea from any cause, not advice written for GLP-1 medicines.
- Eat 6 to 8 small meals a day instead of 3 large ones [17].
- Choose bland foods such as crackers, toast, rice, potatoes, noodles, or baked chicken or fish [18].
- Aim for 8 to 10 cups of clear liquids a day so you do not dry out [19].
- Stay away from foods with strong smells, and from caffeine, alcohol and fizzy drinks [20].
- Sit up after you eat instead of lying down [21].
Advice written for GLP-1 treatment says much the same. A 2025 joint advisory from four obesity and nutrition groups (the American College of Lifestyle Medicine, the American Society for Nutrition, the Obesity Medicine Association and The Obesity Society) says nausea often comes in the morning or after long stretches without eating. It suggests smaller, more frequent meals and avoiding fatty or high-fiber foods in the first few days of treatment [27].
When to call a doctor
MedlinePlus says to contact your provider if you cannot keep food or liquid down, vomit three or more times in a day, or have nausea for more than 48 hours [22].
The medication guides add warnings that apply to these drugs specifically:
- Dehydration and the kidneys. Nausea, vomiting and diarrhea can dry you out, which can lead to kidney problems; drink fluids and tell your provider right away if the symptoms do not go away [23].
- Pancreatitis. Stop the medicine and call right away if you have severe stomach pain that will not go away, with or without nausea or vomiting; the pain may reach your back [24].
- Gallbladder problems. Call if you have pain in your upper stomach, yellow skin or eyes, fever, or clay-colored stools [25].
- Severe stomach problems. In the Wegovy weight trials, severe gut side effects were reported by 4.1% of people on the injection and 0.9% on placebo [26].
