Fifty 410 review

What it is

Fifty 410 is a telehealth service that sells compounded semaglutide and compounded tirzepatide, and it received an FDA warning letter in February 2026 over how it marketed them [12]. It says it is a platform and does not make or ship the drugs itself; independent compounding pharmacies do [1]. FDA addressed the letter to Aspen Aesthetics, doing business as Fifty 410 [12].

What it prescribes

Its weight-loss menu is compounded semaglutide and compounded tirzepatide [2]. There is no FDA-approved brand-name drug for sale. The pricing page lists semaglutide by the month and tirzepatide as prepaid 3-month programs by dose tier [4].

Where it operates

The main site does not list the states it serves. The only list we found is on an older help-center page on the company’s own domain, dated January 2025 and not linked from the current site. It says “45 states,” but the list is 44 states plus the District of Columbia [17]. Alabama, Alaska, Hawaii, Louisiana, Mississippi and South Carolina are not on it [17]. Because the list may be out of date, our score treats the states it serves as unknown; ask before you fill in the intake, and check that your state is covered.

Signing up and cancelling

You do not pay to start. Fifty 410 sends a payment link only after a licensed provider writes a prescription [9]. If you cancel after paying but before the pharmacy starts compounding, you get a full refund; once compounding starts, there is no routine refund [10].

Safety and regulatory record

FDA reviewed the Fifty 410 website in December 2025 and wrote to the company about its compounded semaglutide and tirzepatide [12]. The letter said the product photos showed “Fifty 410” on the label, which suggested Fifty 410 made the drugs when it did not [13]. It also objected to the phrase “clinically proven ingredients,” saying the claim implied FDA approval or review that never happened [14].

FDA’s database listed no close-out letter when this page was updated, so we treat the letter as open. The current home page does say Fifty 410 is not the compounder or manufacturer [15]. Because of the letter, and because it sells compounded drugs, GLPLens does not link to the company’s site.

Who it suits, and who should look elsewhere

This service may suit a cash buyer who has chosen a compounded drug with a prescriber and wants to pay only after approval. Look elsewhere if an open FDA warning letter worries you, if you want an FDA-approved medicine, or if you need to know up front that your state is served.
