Eli Lilly's own pharmacy service for Zepbound and Foundayo, at Lilly's published self-pay prices with no membership. You need a prescription from your own clinician, and the lower price on higher doses depends on refilling within 45 days.

What it is

LillyDirect is Eli Lilly’s own direct-to-patient pharmacy service [1]. It does not fill prescriptions itself: licensed partner pharmacies fill and deliver them [2], and today those are Amazon Pharmacy, Fuze Health, Gifthealth and Walmart Pharmacy [3].

There is no membership. You pay for each fill.

What it prescribes

Nothing: LillyDirect is a pharmacy service, not a clinic. Zepbound needs a valid prescription from a licensed provider [4]. You ask your own doctor to send it to LillyDirect Pharmacy, and if you have no doctor, LillyDirect lists independent telehealth providers [5]. Lilly says it does not endorse those providers, and their listed visit rates do not include the medicine [6].

What it costs

These are Lilly’s self-pay prices per month for Zepbound vials [7] [8] [9] [10]:

The KwikPen starts at the same $299 for 2.5 mg [15]. The $449 price needs each refill bought within 45 days of the previous delivery [11]. A vial month means 28 days and 4 single-dose vials [12], and you buy needles and syringes separately [13].

Foundayo starts at $149 a month for the 0.8 mg dose [20], and 5.5 mg is $199 [22]. Those lower 2.5 mg and 5.5 mg prices are temporary, through December 31, 2026 [21]. The 9 mg dose is $299 [23], and the top two doses keep that price only if you refill within 45 days [24]. A Foundayo month means 30 days [25].

Lilly says its Zepbound products may be HSA or FSA eligible depending on your plan [26]. For every dose over a year, see our LillyDirect cost page.

On insurance, Lilly says Zepbound through LillyDirect and retail pharmacies works for insured and self-pay patients [16], and it checks your coverage and helps with prior authorization [19]. With commercial coverage, the single-dose pen can cost as little as $25 a month [17], but people with government insurance are excluded from that offer [18]. Vials are self-pay only [14].

Signing up and cancelling

There is no membership to cancel. You pay per fill, and we found no auto-refill or subscription terms on its pages. The one rule to know is the 45-day refill window on the higher Zepbound doses [11]: miss it and the next fill costs more [10].

Safety and regulatory record

Who it suits, and who should look elsewhere

LillyDirect suits anyone who already has a prescriber and wants Zepbound or Foundayo at Lilly’s own price, without paying a program membership on top. Pair it with a clinician you trust; many programs in our reviews send prescriptions here.

Look elsewhere if you want the care and the drug in one place, or if you need Wegovy, which Novo Nordisk sells through NovoCare.
